1. Complete a simple application.

2. Order a paramedical exam including lab tests.

3. Write "CPS Express Case" and your CPS case manager's name on a cover sheet. Then fax or e-mail the application to us. If the applicant has a medical impairment covered under the Express guidelines, fax the application and the supporting questionnaires (e.g. Avocations, Aviation, and Foreign Travel) to (805) 534-2929. If the applicant has an impairment not covered under the Express guidelines, complete a Quick Quote Questionnaire for your client's specific condition and fax it to (805) 534-2929.
